The Portsmouth Community Assessment Team (PCAT) works in partnership with community and urgent care to ensure the citizens of Portsmouth are safely returned to their communities as swiftly as possible.

 

Working as part of the urgent care system across Portsmouth and South-East Hants, your work will impact ambulance release and patient flow through our Acute Trust partner, ensuring those within our care are not left in an acute bed longer than necessary.
