About the Role:
We are seeking to appoint an experienced Social Worker to support our patients who are living with palliative and end of life conditions and to provide on-going support to their families and carers. You will be responsible for the provision of social work services:
About you:
You will hold current registration with the Health Care Professional Council and be qualified to CQSW or Diploma in Social Work or BA (hons) in Social Work. You will ideally have a degree in social work. You will be able to demonstrate your commitment to CPD and maintaining your professional status with the Health Care Professional Council.
You will have experience of providing support services to patients and their families that promote dignity, independence, choice and control whilst safeguarding vulnerable individuals including children.
You will be able to plan, initiate, monitor, develop and provide an outcomes-driven service in Adult and Child Social Care to our patients and their families.
Understanding of relevant legislation/guidance including Care Act, Mental Capacity Act, Deprivation of Liberty and Safeguarding regulations is critical for this role, together with an in depth knowledge of best interest.
You will be able to demonstrate strong communication skills, both verbally and written which will enable you to establish professional, effective working relationships with a range of partners including Children, young people, vulnerable adults and children. You be a confident motivated individual who is able to project warmth, empathy and sensitivity when supporting your patients, their families and carers.
You will be an organised individual who is able to prioritise and manage their own workloads whilst working within a highly emotional and flexible environment.
You will have a full current driving licence and use of a vehicle for business purposes as you will be required to visit, patients, their families and carers together with other professionals outside of the Hospice location.
