Band 5 Community Staff Nurse –

Within the Midlands Partnership Foundation Trust (MPFT) we have a number of opportunities for Band 5 Community Staff Nurses to join our locality; this includes areas covering Stafford, Cannock, Rugeley, Stone and Seisdon. The positions are within the day team Community Nursing, Ambulatory Clinic Team and also Community Intervention Service, and are all full time but part time may be considered. We are recruiting dedicated and compassionate nurses to further enhance the high quality patient experience we offer in the community.
The successful applicant needs to have a commitment to providing care in patients’ own homes and residential homes, undertaking Palliative Care including management of syringe driver medication, Tissue Viability and Pressure Area Care, Diabetes Care, continence care and managing long term conditions, being able to work on their own initiative and work well within a team, demonstrating these wide-range of clinical skills. The community intervention service manage acute short term health needs, preventing hospital admissions and facilitating early hospital discharges, and short term post op care. Ambulatory Clinic teams predominantly proved wound care along with other treatments to ambulatory patients able to attend set clinics across Cannock, Stafford and Seisdon areas.

Community services are managing patients with a more complex acuity than ever before, with increasingly specialist treatment and care being provided by highly skilled staff. For the right applicant, training and development can be provided to meet the individualised needs of patients in our community.
