POST: Social Worker Level 1 - permanent

BASE: Tamworth and Lichfield

HOURS: 37.5

TEAM: Tamworth and Lichfield Community Hospitals.

 

An exciting opportunity has arisen within the South East Unplanned Service for a registered band 5 Social Worker.

The post holder will be a registered Social Worker and will have responsibility for completing statutory assessments to enable timely and safe discharges from Hospitals. Tamworth and Lichfield Community Hospitals are based at Samuel Johnson Community Hospital Lichfield and Sir Robert Peel Hospital, Tamworth. It will be the postholder’s responsibility to work with the multi-disciplinary team to ensure safe and timely Hospital discharge.

We are looking for an enthusiastic, proactive and innovative individual who possess excellent communication and organisational skills and has a good working knowledge of the Care Act 2014, can work flexibly, autonomously and as part of a team.
