£20,330 to £21,777 pa

Wiltshire Health and Care provides community based health services, enabling people to live healthy independent lives. We are an NHS partnership organisation, focused entirely on delivering quality health and care to the people of Wiltshire - not on making a profit.

There has never been a better time to join us. Community services are expanding as part of the NHS Long Term Plan and the population of Wiltshire has never needed the energy, enthusiasm and commitment of our community-focused teams more than now. You’ll join a friendly and supportive team who live our values of Quality, Integrity, Partnership and Change, which were developed with our staff, patients and carers. We have an overall ‘Good’ rating from the CQC, with the majority of our services rated as ‘Outstanding’.

If you already live in or near Wiltshire, you’ll know that it is a great place to live and work: rolling countryside, bustling market towns and vibrant city life combined with excellent transport links and good schools. If you are looking to move to the area, we offer local employment and a chance to play a crucial role as part of your new community straight away. What are you waiting for

Rehabilitation Support Worker - Band 3

Wilton Community Team

30 Hours per Week - Permanent

Could you support patients when they return home from hospital? Are you keen to help prevent people being admitted to hospital? Would you like to be a valued member of our forward-thinking community teams? If so, then come and join our Home First programme.

We have a Band 3 Rehab Support Workers/Therapy Assistants trained to NVQ III standard (or equivalent) or people willing to train to this level. We want to meet applicants keen to develop into this rehabilitation role or who have existing transferable skills.

You’ll be part of the Wilton Community Team, Wilton Health Centre and you will be working across a wider area in our South locality. You will be supported by our physiotherapists, occupational therapists and nurses and will work alongside our Assistant Practitioners and existing Support Workers. You will play a vital role helping people to rehabilitate and regain their independence in their own home.

If you have experience in front line NHS community patient care then that’s great but it’s not essential. We want enthusiastic, motivated and hard-working people who can work flexibly and efficiently. We would like you to have effective communication skills, good IT knowledge, and the enthusiasm to develop.

Wiltshire Health and Care is developing training specific for these roles and will provide you with a supportive, friendly and happy environment in which to learn new skills.

The community teams deliver services from 7am to 10pm, seven days a week. We can talk about your preferred hours of work and working patterns at the interview or you can call the following managers for a chat.
