A fantastic opportunity has arisen within Cambridgeshire County Council and Peterborough City Council’s Continuing Healthcare team. We are seeking to recruit a Senior Continuing Healthcare Social Worker, on a permanent full-time basis.

The role combines practice and development. The successful candidate will hold a small caseload, participate in multiagency panels, quality assure the teams assessments and reviews, supervise staff, support the team manager, and help implement policies and process in relation to NHS Continuing Healthcare, ensuring compliance with legislation and the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care (2018) and the National Framework for Children and Young People’s Continuing Care (2016).

In addition to which the successful candidate will work with stakeholders and staff across the Adult Social Care system to be confident in discussions and the delivery of NHS Continuing Healthcare.

Recognising that this is a specialist role, the successful candidate will hold the following additional responsibilities:

• To act as a panel member for regional Local Resolution panels.

• To undertake observations of practice in relation to NHS Continuing Healthcare and create development plans

• To support the Adult Social Care CHC Leads to develop and support their peers

• To work in partnership with the Clinical Commissioning Group to improve individual’s experience of NHS Continuing Healthcare and Continuing Healthcare

• To ensure compliance with the Local Authorities statutory responsibilities under the Care Act 2014, the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care
(2018) and the National Framework for Children and Young People’s Continuing Care (2016).

• To act as a critical friend within the Health and Social Care system to deliver change and ensure quality outcomes

• To Support the team, providing supervision, our conversation and completing Quality assurance in line with Council guidelines

• To coach and mentor staff within the service to improve their knowledge and skills in relation to NHS Continuing Healthcare

Essential Criteria for Senior Social Worker:
- Qualified Social Worker registered with Social Work England
- Full Driving Licence and willingness to travel for work
- Experience of working in NHS Continuing Healthcare

- Practice Educator Qualification

IMPORTANT: This role will require you to enter the premises of a CQC-registered care home as part of your employment. Regulations which come into force from 11th November will require employees over 18 who enter CQC-registered care homes in England to have completed a full course of authorised COVID-19 vaccine doses at least 2 weeks before 11th November, unless they have a medical exemption. As part of your pre-employment checks you will need to provide evidence of your vaccinations or medical exemption. Your employment will not commence until this evidence is received.
