CSH are offering a full-time position to support the Community Heart Failure Team. This is a small, very dynamic, proactive team that provides both domiciliary and clinic-based care. You will need to have a good background of working with Heart Failure patients at a senior level and be willing to support new developments that help transform the delivery of excellent care in the local community.
