The Dementia / Delirium specialist practitioner will play a key role in the leadership of the frailty pathway to ensure the delivery of quality care. They will take accountability for the delivery of the highest standards of practice and care delivered to patients and carers within the community settings across NWS, working in partnership with Adult Social Care teams and key stakeholders. To secure and aspire to the highest standards of holistic care and support by providing leadership to Multi Professional Teams

You will work autonomously for much of the time, with responsibility for making clinical decisions and judgements as part of the multi-disciplinary team. Working within frailty, you will be required to be involved in planning safe and effective personalised care.

The successful candidates will have access to a range of training, learning and development opportunities, both internally and externally.                                                  Free parking is available on site and a staff canteen
