Moving forwards with an integrated workforce, an opportunity has arisen for an experienced, innovative Nurse who is looking for an exciting challenge. This new way of working offers the opportunity to clinically help shape the development of the Neighbourhood Teams and contribute to key pathways and clinical frameworks, working across the health and social care economy.

About You

The successful candidate will be committed to improving patient care, working within and promoting a case management model, providing visible and effective leadership at the point of care and further developing their knowledge and clinical skills.

This post will suit an individual who is seeking to progress clinically and is therefore willing to undertake the further studies which are required including Independent and Supplementary Prescribing, Advanced Practitioner Studies and disease specific modules/diplomas.

As a clinician and leader at the point of care the role will involve close working with Community Matrons, the wider Neighbourhood Team and GPs to manage complex patients with long term conditions and supporting new initiatives promoting proactive case management to avoid unnecessary hospital admissions.

Due to the nature of the role it is essential that applicants are able to travel independently across the locality and have access to a suitable vehicle for business purposes. If necessary adjustments can be considered in accordance with the Equality Act 2010.
