Job Specific Location/Base - St Mary's Community Health Campus, Portsmouth

We are looking for an experienced and enthusiastic GPhC registered Pharmacist who is keen to work closely with vulnerable patients in their own homes to join the Medicines Advice at Home team.

For this post, you will need to hold a full UK driving licence and have access to the use of a car.

The role supports the delivery of the Portsmouth Medicines Optimisation strategy for patients, often frail, with unmet pharmaceutical needs in the community. It involves carrying out targeted reviews for patients with medication related problems to improve medicines adherence, minimise unnecessary hospital admissions and to reduce inappropriate medicines use and waste. The Medicines Advice at Home team works closely with GP surgeries, community pharmacies and a wide range of other health and social care teams. We welcome applications from Pharmacists currently working in either community or secondary care.

Job Responsibilities

  • Effectively respond to referrals to the Medicines Advice at Home team and undertake home visits to patients on our caselaod
  • Work collaboratively with colleagues within the team and in primary, secondary and social care services
  • Contribute to and manage referrals for inclusion in the CCG funded Concordance support scheme, including those for assistive technology
  • Support the management and leadership of the Medicines Advice at Home team

  • Good communication skills, written and verbal.

  • Good problem solving skills and an abilty to influence and negotiate
  • Ability to work well own your own as well as part of a team.
