Continue your Occupational Therapy career journey in an exciting forward thinking service. We have an opportunity for you to be part of our integrated multidisciplinary team, providing rapid Occupational Therapy assessment and interventions within the Older Adults Acute Care Pathway. The postholder will work across this innovative service model, including the Home Treatment and Inpatient Service.

The role of the Occupational Therapist is highly valued in the team and plays a vital part in the service which aims to prevent inpatient admission with intensive support in the community, and also prepare and support discharge from hospital. You will fully utilise your knowledge and experience of managing complex mental health needs, and understanding of older people’s physical health problems. Your delivery of occupational therapy will be person centred and include both 1:1 and group interventions.

Occupational Therapists within the Trust use the Model of Human Occupation to shape assessments and overall approach, however knowledge of the Cognitive Disability Model and the Pool Activity levels will be beneficial in this setting.
