As a Registered Mental Health Nurse / Registered General Nurse, you will be work as part of a team of experienced and skilled Registered Nurses, including mental health nurses, GP’s, pharmacy, and Occupational therapist. To help improve outcomes for local care home residents. You will need to have the ability to present to wide groups of people and the clinical skills for holistic assessment, and implementation of treatment escalation planning to reduce unnecessary admission to hospital. You will need to be enthusiastic and capable to support and improved health care for care home residents. The role will include supporting the team and care homes with MDT’s to review and assess residents who are new to the home, have complex needs or have recently been discharged from hospital. We are a 7-day service and the aim is to provide timely interventions to prevent unnecessary admissions to hospital.

This opportunity will allow the candidate to work closely with other clinicians to support the residents in care settings to receive an holistic assessment of needs, and future care planning.

You will need to have the experience of identifying areas for quality improvement and the ability to work in collaboration with care homes, primary care, community nursing, adult social care, and the CCG.

The successful applicant will have experience managing the complex needs of patients in a community / primary care setting, with a broad range of advanced clinical, organisational and prioritisation abilities. You will be required to work autonomously, whether independently or in a team. The post holder will need to demonstrate visible and credible clinical skills and mental health experience.
