As a Home First Community Care Worker you will be working in the Home First Service in North Stoke, working in partnership with other health care professionals and external agencies to provide a personalised service to meet individuals’ needs.

The role will include assisting service users who need care and support to remain independent in their homes within activities of daily living such as meal preparation, washing and dressing, bathing or assistance with the administration of medication.

Ideally you will have some experience of working in a similar role or of caring experience in an informal or formal setting, however you will be required to work autonomously or as part of a multi-disciplinary team. We are looking for candidates with a strong work ethic for providing excellent standard of care with the right values for this role.

The service operates from 07:00 to 22:00, 7 days per week, with early and late shifts on a rota basis.

We are looking for individuals who are passionate about achieving the best patient outcome to meet the patient’s plans of care in their own homes. You will be responsible for a defined population of people within the local health economy across Stoke-on-Trent and North Staffordshire.

Due to the nature of the service it is essential that applicants have the ability to travel independently across a wide geographical area - Stoke-on-Trent and North Staffordshire.
