Our Integrated Community Children’s Nursing Service is expanding, and we are excited to announce an opportunity to appointa highly skilled, enthusiastic and forward-thinking Registered Children’s Nurse with a strong background within Community Children’s Nursing and educational settings.

Our established, passionate and dedicated integrated Community Children’s Nursing Service (iCCNS) works collaboratively with children, young people (CYP), families and the wider multi-disciplinary team to ensure safe care is provided within the community wherever possible. We deliver high quality and holistic, child centred care to CYP who require nursing intervention, across Portsmouth and the South East Hampshire area with acute and long term conditions, complex needs, life-limiting and life-threatening illness, including CYP requiring palliative and end of life care.

For this new role as Clinical Team Leader within the iCCN service, the successful applicant will have the opportunity to shape our responsive service as it continually adapts to the ever-changing needs of the CYP & families we care for. As one of the Team Leaders, you will be responsible for overseeing the smooth delivery of the service. This will incorporate working with multiple stakeholders including education, acting as a role model for junior colleagues whilst delivering nursing care. You will lead on care planning, training and multi-disciplinary liaison for CYP within their home, education and the wider community setting.

Your main responsibility within the iCCN service, will focus on providing day-to-day clinical and professional leadership as well as operational management to the Children’s Continuing Care Team (CCCT) and wider service, striving to provide a high quality and efficient service to children and families. You will be accountable for ensuring the delivery of care across the 24 hour period, planning and co-ordinating care packages as commissioned for children who are eligible, in line with the national continuing care criteria.

You will need to demonstrate excellent communication and interpersonal skills and have the ability to lead as a team player, whilst having the initiative to work autonomously and effectively. You require good organisational skills and evidence of continued professional development and will be supported and encouraged to further enhance your existing leadership and management skills. This post requires youhold a full UK driving licence and have access to the use of a car.
