The Role:
You will be assigned to the South Lincs Rural PCN to support the community mental health transformation programme and will work autonomously providing mental health leadership and implementing clinical processes. You will undertake mental health assessments and arrange follow up in Primary Care as appropriate and necessary to provide therapeutic interventions. You will work in the context of local and national drivers providing advanced clinical care for identification of individual needs which essentially supports diagnosis and treatment options.

Ideal candidates will also be qualified to prescribe and support with medication decisions. Opportunities to develop these skills and qualifications can be provided to the successful candidate.
Whilst a full Job and Person specification is attached to this advert, key elements of the role will include (but not be limited to):

• Providing specialist clinical knowledge and interventions to service users in primary care setting. This will include working with the PCN to support people with highly complex mental health needs/serious mental illness.
• Developing and implementing clinical processes that contributes towards the continuous improvement of mental health provision across the Apex PCN (multiple GP practices)
• Providing best practice training, mentorship and guidance to mental health practitioners and other roles within the primary care/ neighbourhood MDT
• Managing presentations of highly complex/SMI presentations working closely with the secondary mental health services, to maintain personalised care plans which focus on what matters to the person and avoid escalation to urgent care pathways
• To perform the duties of an independent prescriber, ensuring that appropriate medication is prescribed, monitored and administered in line with current best practice and the Trust Medicines Management Policy.

The Successful Candidate will:
• Have the qualifications and experience as highlighted in the Person Specification.
• Be an experienced clinician with proven skills in working with people who have a range of highly complex mental health needs, demonstrating practice that is evidence-based and innovative.
• Have some experience of working closely with or within primary care.
• Demonstrate adaptability, excellent communication skills and self-motivation.

The post holder will be encouraged to contribute to service development, and there will be opportunities for CPD and clinical supervision both via psychiatry department and PCN.
