Band 7 Clinical Specialist Nurse – Specialist Stroke Service

North Somerset

An exciting opportunity to be a part of developing a new Integrated Community Stroke Service starting with this pilot project in North Somerset. We are looking to recruit to a range of permanent roles to provide a high quality evidence-based multidisciplinary service including early supported discharge but also longer term rehabilitation in the community. We aim to reduce the length of stay people are in hospital, provide high quality home based rehabilitation in order for people to work towards their goals, reduce disability and improve quality of life. We also aim to provide vital advice and support to prevent further strokes and help people to live well after stroke.

This development is part of the wider BNSSG stroke reconfiguration programme but is an early implementation pilot meaning that all roles will help to look at developing the service model for community stroke care

As a Band 7 Clinical Specialist Nurse for the Stroke Service you will be providing specialised Nursing assessment and implementing patient-centred treatment programmes for people after stroke within their homes or current place of residence as part of the specialist stroke service, working alongside the rest of the MDT. You will be required to provide stroke specific advice and support to other staff in the Integrated Network Teams and wider stroke service as necessary and have line management responsibility to provide leadership, operational support and development and supervision of the multi-disciplinary, integrated, community stroke services.

In this role you must provide professional support and clinical supervision to all relevant practitioners within their discipline across the community stroke service and promote a positive approach to change in order to facilitate the development of community stroke services. You will provide day to day operational management and coordination of resources making sure that the service is responsive to the needs of the service users and their carers and to the changing demands of the local health community. Provide leadership, clinical support and supervision to motivate team members and develop effective team working and personal development. You will also be responsible for triaging referrals & be a key point of contact for the community stroke service.

Education and Qualifications Required:

  • Registered nurse
  • Batchelor’s degree in nursing or health related subjects or equivalent experience
  • Non-medical prescribing course or willingness to complete
  • Evidence of study at masters level or willingness to undertake
  • To be able to travel between a variety of locations within BNSSG. A motorised vehicle is required.

Knowledge and Experience Required:

  • Evidence of highly competent clinical reasoning skills and be able to communicate this to other professionals
  • A high level of effective communication and an ability to develop relationships with patients, carers and staff in other teams and organisations.
  • Able to work collaboratively within own and multidisciplinary health and social care teams

  • A good degree of prioritisation skills and the ability to manage, organise and delegate own caseload.
  • Computer skills including e-mail, word processing, power-point and the use of data bases to search for literature.
  • Highly competent clinical documentation skills.
  • Demonstrable people management skills; you will need to be able to supervise and manage junior staff and students.
  • Significant experience of the core areas of nursing, stroke, neurology and elderly care including:

- Experience in Community Setting

- Experience in the specialist area of stroke

- Experience of day to day supervision of more junior members of staff

- Documented evidence of CPD in accordance with NMC recommendations
