We are looking for an experienced Community Nurse to work with in our Community Nursing Support Service (CNSS) as a Community Matron!

The CNNS team enhances the care that is delivered by the 5 Locality Community Nursing Primary Care Network (PCN) teams which are aligned with the 5 GP PCN Groups in Portsmouth. These teams provide patients with a Multidisciplinary team approach for their health care needs. The Localities comprise of Nursing, Physiotherapy and Occupational Therapy staff, and the Community Nursing Support Service. We are fortunate to have a Specialist Diabetes and Care Home Team aligned with the team. The post holder will work within the CNSS team to manage, co-ordinate and support patients who are end of life and wish to remain in their own home to die. You will manage and coordinate a Nursing Team as part of the role.

Your Leadership as a Senior Nurse will include supporting and supervising your staff and enabling them, to provide high quality, compassionate and person-centered care. Your excellent clinical, interpersonal and leadership skills will enable you to work collaboratively within, primary, secondary, and social care partners.

As a Community Matron within the CNSS team you will prescribe and implement a range of care interventions for the people on your caseload, with an emphasis on avoiding unnecessary hospital admissions. Using your autonomous advanced clinical skills, you will plan care interventions for people with multiple, complex long-term conditions and coordinate and lead the team ensuring that patients receive timely and safe care. You will have responsibility for ensuring all health care needs are met during their end of life phase. The post holder will work to ensure integration of all elements of care, increase patient choice and prevent unnecessary admissions to hospital.

You will have or be working towards the extended and supplementary prescribing qualification.(V300)
