Manchester City Centre & Ancoats (MCC&A) Primary Care Network (PCN) is looking for 3 Care Navigators.
The role will support the ongoing development of services delivered by the PCN and will include development opportunities.
Working hours are predominantly during the day during the week. We are supportive of flexible working. There are some fixed meetings which will require attendance. There will be some evenings and weekend work, but this will be kept very much in balance with normal hours. You will work in a mix of remote and face to face. You will need to be able to work with high degree of autonomy. There will be opportunities to lead on service and quality improvement projects.

The Care Navigators will signpost and support people into services in the community, including Voluntary, Community and Social Enterprise (VCSE) sector. They will be able to refer to over 100 different services across the locality. They are able to support joint visits with other professionals as well as lone visits to help support improving an individual’s well-being in their local community. Key relationships will be with the three practices’ management teams, the PCN clinical and social team and colleagues in Local Care Organisation.
