Continuing Care Service for Greater Nottingham is looking for an experienced Paediatric Nurse to join the team as a Nurse Case Manager – Band 7

The Case Manager will be active in the implementation of new ways of working and commission of health care packages at home and other community settings to prevent hospital admissions. In addition they will help facilitate early discharge from hospital and prevent re admissions.

The post holder will be required to commission a high standard of care for children and young people with complex health needs, life threatening or chronic illness. Negotiating effective care packages with children/young people and their families/carers, which meet the requirements of the individual.

The role will include case management of children and young people assessed eligible for continuing health care funding.

You will also be required to implement and assist in the coordination of multi-agency packages of care with health, social care and education and support the continuing care assessor in day to day management of the service.

The Childrens Continuing Care Case manager will ensure that the health care needs of children and families eligible for Children’s Continuing Care are identified and met in accordance with the National Framework for Children and Young People’s Continuing Care 2016.
