An opportunity has arisen for a senior occupational therapist to join our Reablement Discharge to assess and Non-weight bearing Community beds team. The team provide a community bed based rehabilitation service across community care home settings in the Nottingham City locality, often patients are admitted to the community beds directly from hospital but some are admitted directly from their own homes after experiencing a crisis that does not require acute secondary care. This position will form part of the non-weight bearing team who are based within the care homes and complete assessments, rehabilitation and discharge planning in a timely manner to support patients back into the community.

The applicant will have a good knowledge of community services and an understanding of health conditions, especially those associated with orthopaedics and ageing. You will work closely with Nurses, other Occupational Therapists, Community Care Officers, Assistant Practitioners, Rehabilitation Support Workers and Physiotherapists as part of a Multidisciplinary team and have developed strong leadership skills in supporting a team.

The successful applicant will be responsible for providing clinical assessments, identifying rehabilitation goals, undertaking treatments, referring to other services and supervising junior staff. You must possess excellent interpersonal skills and have experience of working with a range of people. Experience of working in a team and the ability to use your own initiative, as well as being an excellent communicator are also essential.

You will possess a minimum BSc Hons/Diploma in Occupational Therapy and be HCPC registered. There will be an induction programme and on-going training is an essential part of the post.

This role involves covering the service on a rota basis from between 8am to 8pm, including weekends and bank holidays.

Please note a car driver with own vehicle is essential for the job role.
