Job Specific Location/Base - St Mary's Hospital, Milton Road, Portsmouth. PO3 6AD.

You will be an experienced / qualified Nurse with a thirst for knowledge and have some understanding of the complex patient group with a diagnosis of heart failure. We are looking for candidates who hold a professional registration with the NMC. For this post, you will need to hold a full UK driving licence and have access to the use of a car.

The Solent is an established team, embedded in Specialist Services and Community Services in Solent. It has clearly defined aims of service, referral pathways and discharge processes.

The Heart Failure Service provides evidence-based support to adults aged 18+, with a formal diagnosis of left ventricular systolic dysfunction (LVSD) who are registered with a Portsmouth City General Practitioner. It provides advice, medication optimisation, education and symptom management, in line with local and National guidelines, to avoid episodes of decompensation and avoid hospitalisation where appropriate. It is not an emergency service but will respond in an appropriate and timely manner.

Following a formal diagnosis of heart failure, we aim to empower the patient, working with them and their care givers, to optimise symptom management using a flexible holistic approach. Contacts and support utilise various mediums such as face to face visiting at home, face to face clinic consultations and telephone support via a Single point of access.

The service acts as an expert resource in Heart Failure management for other generic services needing advice and guidance for symptom management and medication management, through education and support.

The service is hosted by Solent NHS Trust working in partnership with Southern Health NHS Foundation Trust and Portsmouth Hospitals University Trust.The Commissioning organisation is Portsmouth CCG.This Nursing Service operates on a Monday to Friday, office hours basis.

Your priorities will include;

To meet the needs of patients and their families to achieve improved health outcomes and promote health and wellbeing while demonstrating care, compassion, competence, communication, courage, and commitment. Care is based on comprehensive assessment, ensures continuity and is patient/client centred, culturally sensitive and evidence-based and which promotes independence.

Work as an Independent Non-Medical Prescriber to prescribe medication (once qualified to do so) according to patient need adhering to the guidelines from the National Prescribing Centre, NICE and Solent NHS Trust Policy. Or alternatively, as part of the management of complex symptom control provide advice on the tailoring of pre prescribed medications in response to changing clinical need.

Critically analyses complex clinical data and information to inform diagnosis and decision making to offer advice on symptom control utilising appropriate medications according to best practice in specialist heart failure care. Evaluation of the effectiveness of their recommended management and negotiating with healthcare professionals regarding its implementation is on-going.
