In an exciting collaboration between University Hospitals Southampton Foundation Trust and Solent NHS Trust, we are introducing a new Children’s Hospital @ Home team (CH@H) to provide care in the community for Children and Young People with acute and short-term conditions.

This newly commissioned service will operate across Southampton and South West Hampshire (New Forest and Southern Parishes) and will be delivered as a collaboration between Solent NHS Trust, the paediatric team at Southampton Children’s Hospital and primary care.

In response to the many national drivers for children and young people to receive care close to home as well as the increasing admissions of those with acute care needs, this targeted service will work closely with the Children’s emergency department, paediatric wards, CCN team and primary care to facilitate acute assessment, home follow up and supported discharge. The Children’s Hospital @ Home Team will work together to deliver a specialist service to support children, young people and families receive care in the most appropriate location depending on their acute/short-term health needs. The service will operate 365 days a year from 10:00-22:00. This is a bank position meaning hours are adhoc.

Joining now will give you a unique opportunity to help shape and develop the service from the outset, as we prepare for launch in Autumn/Winter 2021.

We are looking for children’s nurses with significant experience of working in acute paediatrics and the confidence to work autonomously to assess children with acute and short term conditions. Experience of working in the community would be advantageous although not essential.
