Healthcare Support Worker, Discharge Lounge – Band 2: 20 hours’ per week (1 post)

Operations

An exciting opportunity has arisen for a highly motivated and committed HCSW to join the Discharge Lounge.

The Discharge Lounge coordinates the patient’s journey back into community and provides a seamless transfer of care /discharge to ongoing primary care services/placements and patients own homes.  

On the Discharge Lounge you will work as part of a team to ensure patients are discharged safely and will be key in supporting the Trusts efficient flow. You will pick up patients for discharge throughout the day, orientating them to the unit and maintaining their health and wellbeing until discharge preparations are complete. As part of our team you will ensure effective communication between care teams and families so providing a safe and timely discharge from hospital.

The cost of undertaking a DBS disclosure will be met by the individual. This will be deducted from your first month’s salary. The current price of a standard check is £23 and an enhanced check is £40.
