Are you a qualified District Nurse who is looking for a new challenge? Do you have an interest in the development of an integrated health care team? Do you want to be involved in supporting the implementation of the Framework for Enhanced Health in Care Homes? If so this post is for you.

We are looking for a Qualified District Nurse Team Leader to join Nottingham CityCare Partnership’s Integrated Care Homes Service. The team comprises of health professionals who in reach into Care Homes within Nottingham City to deliver NHS services, and includes community nurses, dementia outreach staff, specialist nurses, therapy staff and advocacy who work alongside each other to provide integrated holistic care. The service works in partnership with care home staff, pharmacists and GP’s.

The aim of this innovative service is to ensure that care home residents and staff have an improved experience through high quality care and access to familiar health professionals with the skills to meet their needs. A single point of access facilitates a streamlined referral route for the care home staff ensuring that the patient is triaged and allocated to the most appropriate professional to meet their need. The service also delivers training within care homes to equip the care home staff with the knowledge and skills to deliver the programmes of care prescribed by healthcare professionals, by the implementation of innovative ways of working.

The Community Nursing element of the service provides nursing support to patients living in Residential Care Homes within Nottingham City.

As a District Nurse Team Leader you will be part of the senior clinical team within the service. You will have day to day leadership and management responsibility for a team of nurses and assistant practitioners, along with managing a caseload of patients. Using your up to date clinical knowledge and shills you will be required to work autonomously and use your initiative to deliver nursing and holistic care within a supportive team environment.

As a member of the team, you will work in partnership with care home staff, GP’s, and the wider MDT and will be actively involved in the ongoing development of the integrated service. You will be supported to develop the new skills required for this unique role.

The service works in partnership with the Nottingham and Nottinghamshire Clinical Commissioning Group in the development of the care home staff to enable the residents within care homes to receive care tailored for their needs and to reduce preventable hospital admissions.
