An opportunity has arisen for a RGN to join our community beds team. The team provide a community bed based rehabilitation service across two community care home settings in the Nottingham City locality, often patients are admitted to the community beds directly from hospital but some are admitted directly from their own homes after experiencing a crisis that does not require acute secondary care. The team are based within the care homes and complete assessments, rehabilitation and discharge planning in a timely manner to support patients back into the community.

The applicant will have a good knowledge of community services and an understanding of health conditions, especially those associated with ageing. You will work closely with Physiotherapists, other nurses, Community Care Officers, Assistant Practitioners, Rehabilitation Support Workers and Occupational therapist as part of a Multidisciplinary team and have developed strong leadership skills in supporting a team.

The successful applicant will be responsible for providing clinical assessments, identifying rehabilitation goals, undertaking treatments, referring to other services and supervising junior staff. You must possess excellent interpersonal skills and have experience of working with a range of people. Experience of working in a team and the ability to use your own initiative, as well as being an excellent communicator are also essential.

You will possess a minimum BSc Hons/Diploma in Nursing and be NMC registered. There will be an induction programme and on-going training is an essential part of the post.

This role involves covering the service on a rota basis from between 8am to 4pm, including weekends and bank holidays. Also involves working occasional late shifts between 12pm and 8pm.

Please note a car driver with own vehicle is essential for the job role.
