We are excited to be recruiting a qualified Advanced Clinical Practitioner with the master’s qualification including Independent and Supplementary Prescribing. Although employed by Leeds Community Healthcare the successful candidate will be deployed to work as an embedded member of the West Leeds Primary Care Network which comprises 7 practices working together to deliver care for their patient population. The role will support the delivery of a comprehensive frailty service working closely with the local Neighbourhood Team in line with the integration agenda.

You will need to evidence in your application and demonstrate at interview consolidation of your learning together with further formal learning regarding common Long-Term Conditions in order to manage patients proactively and acutely in both GP surgeries and their own homes, working to promote quality of life, avoid inappropriate admissions to hospital and support earlier discharges.

There is a statutory requirement for staff in this role to provide nursing or personal care within Care Quality Commission (CQC) regulated care homes.

Therefore, in line with new government regulations which come into force on 11 November 2021, anyone employed in this role will need to evidence they have had two doses of a Medicines and Healthcare products Regulatory Agency (MHRA) approved COVID-19 vaccine, or that they come within a specified exemption. Evidence of vaccination status should be provided via the NHS app, the NHS COVID pass via the NHS website, or an NHS letter as part of pre-employment checks.
