Self Management facilitators provide a programme of interventions in the Neighbourhood Teams. The overall aim of the service is to work with patients to increase their knowledge, skills and confidence to manage their long term conditions and care needs.

This is a clinical role and will involve care delivery with a behavioural change focus. Examples of care delivery that may be delivered are:

Insulin administration
Catheter care delivery
Simple wound management
Medication administration

The Leeds Approach is to support clinicians to have better conversations with people who live with long-term conditions, based on agreeing goals and actions. This approach underpins the Self-Management Facilitator Role by embedding personalised and patient centred care into all interventions aiming to maintain independence and working with patients and carers in order that they have the opportunity to make the most of life and remain in their home environment for as long as possible.

A programme of intervention is presently being delivered by Self-Management Facilitators working within Community Neighbourhood Teams to empower and motivate patients to manage elements of their own health care needs based on the principles of health coaching, better conversations and motivational interviewing. We are now seeking to find further suitable candidates for these posts.

There is a statutory requirement for staff in this role to provide nursing or personal care within Care Quality Commission (CQC) regulated care homes.

Therefore, in line with new government regulations which come into force on 11 November 2021, anyone employed in this role will need to evidence they have had two doses of a Medicines and Healthcare products Regulatory Agency (MHRA) approved COVID-19 vaccine, or that they come within a specified exemption. Evidence of vaccination status should be provided via the NHS app, the NHS COVID pass via the NHS website, or an NHS letter as part of pre employment checks.

Due to the nature of the role it is essential that applicants are able to travel independently across the locality and have access to a suitable vehicle for business purposes. If necessary adjustments can be considered in accordance with the Equality Act 2010.
