The Neighbourhood Night Service is a forward thinking, dynamic team that is continuously looking to improve and develop as the demand for the service we deliver changes. We are also part of the Virtual Ward (Frailty), which provides coordinated rapid care to people in their home who are experiencing an acute medical episode. We are seeking to appoint a Senior Nurse to work as part of our nursing team to ensure that we continue to deliver outstanding patient care.

The Neighbourhood Night Service operates overnight across the whole of Leeds providing care for patients requiring nursing intervention in their own home. The service is co-located with other community services and operates centrally from Killingbeck Court, Leeds LS14 6FD. Key aims of the service are to prevent unnecessary hospital admissions and ensure that palliative patients can remain in their own homes, where this is their preference. The Neighbourhood Night Service is part of a wider multi-disciplinary team and ensures that there is 24/7 nursing care for patients in the community.

There are three main functions of the Senior Nurse operating within the night service during the night;

- To clinically coordinate and manage care to ensure that all patient calls are effectively assessed, triaged, allocated and recorded. You will need to have an overview of the service as a whole to ensure that both patients and staff remain safe and care is appropriate and effective overnight. For example, re allocating work to cover unexpected staff absence. (The times for the Coordinator shifts are 20.45pm until 07.15am)

- To deliver clinical care to patients and support our Nursing Assistants overnight (The times for clinical shifts are 20:45pm-07:15am)

- Provide clinical leadership, including line management of band 5 Staff Nurses within your area of responsibility and oversee line management of our band 3 Nursing Assistants who are managed by the Staff Nurses.

The Senior Nurse plays a key role in ensuring that communication is effective to the staff on duty overnight and that any matters arising overnight are communicated effectively to relevant parties, which include the day function of the Night Service.

You will have a full tailored induction programme, peer support and on going support from the Night Service Clinical Quality and Operational Leads, Ongoing development is encouraged.

The role predominately covers 20:45pm-07:15am, seven days per week. The contract is for up too 35 hours with night time enhancements (hours available can be discussed prior to application or at interview as required).

There will be an expectation that you will occasionally perform this role during day time hours (07.00am-15.00pm or 14.00pm-22.00pm shift pattern across seven days) to support service delivery and to have an awareness of the way in which the service operates in its entirety.

Due to the nature of the role it is essential that applicants are able to travel independently across the locality and have access to a suitable vehicle for business purposes. If necessary adjustments can be considered in accordance with the Equality Act 2010.

You will be a registered staff nurse and have excellent assessment, planning and evaluation skills in order to ensure that planned patient care is timely and appropriate.

You must have excellent communication, organisational and IT skills as you will be coordinating care for a large number of complex patients.

This is a fast paced role requiring safe and effective clinical decision making, so you must be able to think on your feet, problem solve issues that arise overnight and juggle conflicting priorities. You must be able to motivate others in challenging times and utilise the resources available to you effectively.

There is a statutory requirement for staff in this role to provide nursing or personal care within Care Quality Commission (CQC) regulated care homes.

Therefore, in line with new government regulations which come into force on 11 November 2021, anyone employed in this role will need to evidence they have had two doses of a Medicines and Healthcare products Regulatory Agency (MHRA) approved COVID-19 vaccine, or that they come within a specified exemption. Evidence of vaccination status should be provided via the NHS app, the NHS COVID pass via the NHS website, or an NHS letter as part of pre employment checks.
