We have exciting full-time/part time opportunities for Experienced Advanced Clinical Practitioners in our Greater Manchester based GP practices, due to expansion of services and an MDT approach.

About the Job
The successful candidates will be qualified to MSc level in Advanced Clinical Practice and can be from any HCP background.

As an Advanced Clinical Practitioner, you'll see, treat and discharge patients of all ages, either over the phone or face-to-face or home visits. You’ll be responsible for apply clinical reasoning to assess, diagnose, and prescribe where appropriate. In addition, you will be able to make appropriate referrals and providing the right care and advice for patients with undifferentiated illness and be familiar with EMIS and DOCMAN. You’ll work alongside GP’s, Practice Nurses, Pharmacists and HCA’s and other non-clinical staff to deliver the best possible patient care.

You will support and contribute to the delivery of policy and procedures, provide leadership as required, will act as a mentor and positive role model to other staff and will facilitate training and education as required.

You will have the support of the Primary Care Clinical Lead and our Primary Care Matron who work in partnership with the Director of Nursing and Allied Health professionals. Heads of Locality and Senior Operational Managers, to drive forward clinical excellence, leadership, quality, safety and high standards.

Our permanent roles can offer part and full time up to 37.5 hours per week, between the hours of 8-6.30 Monday to Friday

What we'll need from you
• Be an autonomous Clinical Practitioner with a patient assessment qualification and recent work experience within a Primary Care environment
• Hold an independent prescribing qualification
• Be part of a regulatory body such as NMC and hold the relevant Pin
• Be able to see, treat and discharge patients of all ages with scheduled/unscheduled presentations
