Post: Rehabilitation Support Worker Base: City WideSalary: Band 3

Contract: permanent & fixed term, full and part time

Citycare’s health reablement and urgent care service provides care and rehabilitation for people presenting with complex conditions, as well as challenging medical and social circumstances. We are seeking compassionate, caring and dedicated rehabilitation support workers with a sound knowledge, drive and experience of working with complex, frail and older people. Due to an increase in activity we are seeking additional team members.

You will be working in a supportive, caring team who will provide the necessary knowledge, training, compassion and shadowing to ensure you feel confidents in your role. The hours of work will be 7am- 10pm on a fixed rolling Rota basis (out of hours operations* carries an enhancement as per AFC). The Service operates 7 days per week, 365 days per year and bank holidays. Days are typically 7.5 hours long and your post will be salaried, a full time colleague will work 5 shifts in a 7 day period, staff are paid for mileage during their shift as per agenda for change.

Rehabilitation support workers typically visit patients in their own homes, or in a care home setting, with a view to preventing unnecessary hospital admissions, promoting independence and adding value and quality to life. You will be working closely with, and fully supported by our clinical team to achieve a shared objective which will include conducting exercises or supporting with necessary care goals as per a written care plan.

To apply for this job you must have a full driving license and access to a car for work purposes because you will be travelling between visits, you will not be considered if you do not have the above. As well as carrying out visits you will provide feedback to the wider team both verbally and electronically and we will provide the necessary equipment for this.
