At NDHT our Community Nursing team are on a mission, a mission to keep patients out of hospital and safely cared for in their own homes or within a community setting. Working with a team of healthcare professionals from a wide range of disciplines, you’ll provide the nursing expertise that is essential to patient care and well-being. Our community nurses value the relationships they develop with patients and their families, as well as the opportunity to see first-hand the difference their care makes to a patient’s journey.

These are dynamic and exciting roles where no two days are the same and you will need to think on your feet and adapt quickly, using your expertise to deal with any situation that comes your way.

You will act as an autonomous practioner, which is the speciality of the community nursing role. This will involve mainly working on your own, however you will be well supported by a dedicated and highly motivated team, and a strong investment in your continued training, development and career progression

As this role involves working in a Care Home, please note you need to have received both doses of the COVID vaccine or be medically exempt to apply for this position due to the government legislation that came into force on 11th November 2021 that requires all persons working in Care Homes to be double vaccinated.

Our teams of care professionals work with patients and their families to support peoples’ independence, health and wellbeing. We provide support to avoid hospital admissions, and if an admission is necessary, we try to make each patient’s stay in hospital as short and effective as possible before working with them on a safe discharge home.

In our most recent staff survey, our staff voted us in the top 10% of NHS provider organisations to work for and we also came out as the top provider in the South West for our overall results. Our patients (through the Friends and Family Test) regularly report an average of over 95% satisfaction with our services.
