In an exciting collaboration between University Hospitals Southampton Foundation Trust and Solent NHS Trust, we are introducing a new Children’s Hospital @ Home team (CH@H) to provide care in the community for Children and Young People with acute and short-term conditions.

As a Children’s Nurse within the CH@H Team you will have the opportunity to shape a new and responsive service as it continually adapts to the ever-changing needs of the children, young people & families we care for.

This newly commissioned service will operate across Southampton and South West Hampshire (New Forest and Southern Parishes) and will be delivered as a collaboration between Solent NHS Trust, the paediatric team at Southampton Children’s Hospital and primary care. In response to the many national drivers for children and young people to receive care close to home as well as the increasing admissions of those with acute care needs, this targeted service will work closely with the Children’s emergency department, paediatric wards, CCN team and primary care to facilitate acute assessment, home follow up and supported discharge. The Children’s Hospital @ Home Team will work together to deliver a specialist service to support children, young people and families receive care in the most appropriate location depending on their acute/short-term health needs. The service will operate 365 days a year from 10:00-22:00. Joining now will give you a unique opportunity to help shape and develop the service from the outset, as we prepare for launch in Autumn/Winter 2021.

We are looking for experienced Children’s Nurses at bands 5, 6 and 7 to recruit to this new team within our integrated Community Children’s Nursing (CCN) service.

The CH@H team will be overseen by Consultant Paediatricians and Clinical Matron, it will consist of a clinical team leader, senior children’s nurses and experienced children’s nurses.

The wider CCN service consists of passionate and dedicated CCN, Specialist School Nursing, Asthma, Continence and Continuing Care teams who work collaboratively with children, young people (CYP) and families within the wider multi-disciplinary team to ensure care is provided within the community wherever possible. We provide high quality and holistic, family centred care to CYP with nursing needs across the Southampton and West Hampshire area including those with;

· Acute and short-term conditions

· Long term conditions

· Disabilities, complex conditions, those requiring continuing care and neonates

· Life-limiting and life-threatening illness, including those requiring palliative and end of life care

We are looking for children’s nurses with significant experience of working in acute paediatrics and the confidence to work autonomously to assess children with acute and short term conditions. Experience of working in the community would be advantageous although not essential.

As a band 7 Nurse you should hold the CCN Specialist Practitioner Qualification or equivalent in Advanced Clinical Practice, have evidence of Masters level study/qualification, have experience of clinical leadership, caseload management, line management and supervision as well as significant experience of teaching/supporting learners in practice.

This opportunity will provide experience of working with CYP and families in the community setting as well as the chance to transfer a wide range of clinical skills. This role will also enable you to further develop your leadership and management skills within the team. If you have excellent communication and interpersonal skills, the ability to lead as a team player, as well as the initiative to work autonomously and effectively this could be the challenge for you. You should have good organisational skills and evidence of continued professional development.
