Solent NHS Trust have an exciting opportunity to join the Community Intensive Rehab team (CIRT) as Clinical Manager. You must be a qualified Registered Social Worker or Occupational Therapist with experience of working in mental health

CIRT aims to ensure that people receive rehabilitation when they need it in the community and promotes a positive long-term approach to recovery.

The CIRT can work in all community settings; though will commonly work with people living in supported accommodation, often long term, in order to enable their optimum level of functioning and independence. CIRT will support people to develop structure and purpose in their day to day lives and promote a sense of community and belonging.

The CIRT is embedded within the Solent NHS mental healthcare system. It sits alongside the Community Mental Health Recovery Teams. People supported by the CIRT will receive care co-ordination and medical input from the Community Mental Health Recovery Teams; the CIRT will work very closely with the recovery teams, acute care pathway, providers in supported living environments and others within the persons care network e.g. GP.

Scope and Definition - Who do we work with?

Portsmouth residents; aged over 18 that have complex psychosis and related severe mental health conditions; a primary diagnosis of a psychotic illness (including schizophrenia, bipolar disorder, psychotic depression, delusional disorders and schizoaffective disorder) plus severe, treatment-resistant symptoms (positive and/or negative) and/or coexisting mental health problems such as severe anxiety, depressive or obsessive compulsive symptoms or substance misuse.

The successful applicant will lead this team and have a good knowledge of the housing pathway including the out of area high cost placements. You will need to be highly motivated, passionate and enthusiastic. The position requiresexcellent leadership skills.

Key responsibilities include: supervision, appraisals, performance management, recruitment and general day to day management of the team ensuring people have timely access to our service.
