Referral Support Administrator

Surrey Heartlands CCG are looking for an enthusiastic Referral Support Administrator to be

  • Responsible for ensuring the efficient and accurate and responsive processing of referrals sent to the Referral Support Service from GP surgeries and other health care professionals
  • Providing support to referring clinicians on service criteria, operational processes, pathways and protocols
  • Supporting patients through their personal Choice of Provider, along with other referral queries
  • To proactively participate in meetings and contribute towards effective working and team innovation
  • Provide support and learning for team members to enable patient referrals in line with operational processes, pathways and protocols
  • Use initiative when faced with situations requiring a problem solving approach
  • The post holder will be dealing with frequent interruptions and competing demands from staff or work related queries

We have an ambitious vision for transformation and continuous improvement of health and care across our footprint. We are in the unique position of having secured a devolution deal, which for the first time really brings together health and social care by allowing us to commission from a single budget. We are also developing four dynamic and sustainable Integrated Care Partnerships (across East Surrey, Guildford and Waverley, North West Surrey and Surrey Downs localities) based on co-operation with our component partners, and focusing on local population needs, priorities and decision making.

In addition, we have responsibility, delegated from NHS England, for managing Primary Care contracts across the Surrey Heartlands footprint
