If you are looking for a change, a new challenge and career development…this may be the opportunity you are looking for.

We are looking for a caring and compassionate qualified District Nurse (possessing Specialist Practitioner Qualification) or a Senior Community Nurse who is keen to progress professionally towards the Specialist Practitioner Qualification. You will lead a community nursing team within a multi professional environment. To excel in this role you will require excellent communication skills, the courage to make decisions which will help shape future care and a willingness to develop your potential. You will require community nursing experience, a sound knowledge of evidence based community care and best practice, evidence of post-registration development, the competence and confidence to advocate for patients regarding complex situations and the ability to work over a shift system which delivers service between 8am and 8pm across 7 days, including weekends and bank holidays.

We seek nurses with passion and commitment who hold patient experience and care at the heart of their nursing practice and share our organisational values. In return we offer you day to day support along with excellent training and a real commitment to your personal development plan with opportunities for continued professional development and study. You will be joining us at an exciting time, as we move towards a model of ‘named nursing’ in the community- enabling person-led, progressive care. We are a fully integrated service working with MDT colleagues in clusters to deliver care around the placed based model fit for the future.

We are a modern trust, which means that you’ll be equipped for mobile working (using a laptop and smart phone); not only is this safer because you will have access all patient information at your fingertips, it also means you can complete records and any other patient paperwork in real time, so you can then focus all your attention on your next patient.

Please ensure when applying for this position that, in your supporting statement, you make it clear as to how you meet the person specification especially the values and behaviours expected of all colleagues (for example evidence of how you meet our Trust Core Values). This will figure largely in our short-listing criteria and the overall selection process.
