Integrated Cluster Service Networks Teams

x1 WTE post will be for District Nursing West Network

x 1 WTE post will be for District Nursing East Network


This provide an exciting opportunity for highly motivated and ambitious staff who are committed to develop excellence and leadership in district nursing and also providing vision and energy in developing holistic integrated care and support for the patient and those who are important to them. The role requires effective management and strong leadership qualities. The post holders, will be skilled and creative and will work as an integral manager in an Integrated Cluster Network Team. The post holders will need to work closely with GPs and surgery staff, and the wider multi-disciplinary staff to support individuals to have good quality emotional and physical well-being in their own homes.

The post holder will offer clinical leadership within the Network Team and provide support and professional development for their staff. They will oversee the operational management of caseloads ensuring high quality care for patients and support for those important to them. The post holder will support the Integrated Cluster Network Service Manager in developing the Team’s multi-agency approach to patient care. This will be integral in all that they do.

The Integrated Community Network Teams work to develop seamless pathways of care and support. This role will be key in being responsive to people’s needs. The role will be key in bringing in a multi-disciplinary approach to provide and implement timely professional support for people’s needs. This ensures choice and control for individuals and places the person at the center of their care. It provides a clear expectation that the person’s voice and choice will be heard. The role with concentrate on maximizing the independence of adults and enabling them to live their lives as they wish. The Teams are ambitious in looking to develop innovative and creative ways to support the individual and those important to them. It works to recognise the changing needs within communities and works collaboratively to meet them.

The Team consists of staff from District Nursing; Social Work; Occupational Therapy; Physiotherapy; Community Psychiatric Nursing; Speech and Language Therapy; Dietetics and Pharmacy and links closely with the Third Sector. This holistic approach provides a full offer of support for adults with changing needs in their localities as well as meeting complexity of needs. Acting within their professional boundaries, the post holder will provide support to the person they are working with and those important to them through holistic person centered practice. They will be a key member in providing an effective team around the person.
