Our shift patternsoffers a good work life balance:

  • Core: - 08:30 – 16:30
  • Mid Shift – 10:00 - 18:00 or 12:00 20:00 (average 4 per month)
  • Twilight – 14:00 - 22:00 (average 4 per month)
  • Weekend working is 1 in 4

We are looking for full and part time applications with a minimum hours of 22.5 hours per week.

We are seeking nurses to provide complex care and treatment to both adults and older people living at home who have long-term or complex health needs. You will work alongside specialist community services including Palliative Care, Proactive Nursing Services, Bladder and Bowel, Respiratory, Diabetes as well maintaining established pivotal relationships with our primary care colleagues.

In this role you will build long-term relationships with patients providing them care in the comfort of their own home. You will work as part of a motivated and caring team who deliver exceptional high-quality healthcare, which keeps our patients at the heart of everything we do.

In this varied role your responsibilities will include:

  • First contact: acute assessment, nursing diagnosis and care, treatment, and referral
  • Palliative care: symptom management including assessment of acute symptom onset, care for the patient and their family/carers
  • Continuing care: intermediate care, rehabilitation, chronic disease management
  • Public health: health protection and promotion programmes that improve and reduce inequalities
  • To actively safeguard and promote the welfare of children and adults

About you

We are looking for applications from either experienced nurses or newly qualifed nurses who are due to graduate within the next 6 months.You will be dedicated, highly motivated, and committed to providing high standards of care to people within their own homes and the local community. You will have excellent communication skills as you will be undertaking complex nursing assessments, you will also be required to build relationships within the specialist teams to help deliver a high-quality service.

Experience in community nursing is desirable for this post but not essential as full training will be provided, and the post holder will be supported to develop in the role.

You must have a full UK driving license and access to a car for work purposes, you will have the opportunity to apply for a lease car.
