Surrey Heartlands CCG are looking to recruit a Care Home Digital Project Facilitator to support the deployment of digital projects into Care Homes across the Surrey Heartlands footprint. This role is required to specifically support the deployment of a digital project involving the remote monitoring of patients in Care Homes with long term conditions. Working with the Project Team this role would involve visiting Care Homes to give advice and support to Care Home staff and assist them in their adoption and sustained use of the monitoring software, so travel will be required. Full training and support will be given.

The ideal candidate for this opportunity will be a motivated, enthusiastic and flexible individual who has good communication and organisational skills, being able to work independently and as part of a team. A good understanding of IT and the ability to pass on information and instruct others in the use of software and devices is essential. The successful candidate will need to be able to communicate effectively with a wide variety of people including health professionals, Care Home and GP Practice staff and CCG colleagues.
