Surrey Downs Health and Care Partnership has a clear ambition to create a health and care system built around the people and communities of Dorking, Epsom and East Elmbridge and is continually evolving through system wide collaboration and co-creation. To achieve this requires transformational change in the way services are planned and developed with a commitment to co-design and a focus on outcomes for the individual and community, not the organisation.

We are looking for a Programme Manager to join the team on a 6-month fixed term contract. The post holder will play a leading role in redesigning care pathways ensuring high quality, joined up and effective care is provided to the local population in the most appropriate place at the most appropriate time. The post holder will be responsible for the development and delivery of key programmes of work in line with Surrey Downs Health and Care Partnership model of care and transformation priorities and provide operational and programme leadership to support the delivery of change across the system.
