An exciting opportunity has arisen for a band 4 Clinical Triage Officer within SPUR/Gateway and Police Custody Healthcare. We are a Clinical and Customer Service Team employed to process referrals for a variety of different customers. We are currently based in Leeds City Centre, working in partnership with Leeds City Council to ensure the best care for our patients.
The service is the first point of contact for professionals, for signposting to appropriate departments. We process referrals for people who are either living in, or have a GP in Leeds who require a health or social care service. In addition to this, we also take telephone healthcare referrals for individuals in Police Custody suites throughout Yorkshire and Humberside.

The candidate will report directly to the Team Manager, and will be a point of contact between members of the SPUR team, neighbourhood teams and external agencies in relation to new and existing referrals. To be able to deal effectively with any enquiries, advice or new referral calls. In addition to this the candidate will assist and support the Triage Coordinators in SPUR to ensure the efficient and effective functioning of the SPUR/Gateway team.

The candidate’s main duties will be to screen referrals in a timely manner being able to prioritise based on urgency. The candidate will be involved in the day to day running of the office – maintaining good communication between CSO’s, Clinicians and management.

The post holder will be required to support CSO’s by helping to process referrals during periods of high demand, and will be involved in the training of CSO’s and implementing process changes within the service. The post holder will coach staff on their calls and referral to make sure they are complying to policy and delivering the right customer service we expect.

The successful candidates will have experience of working in a clinical environment. Clinical knowledge of the adults and complex health conditions is essential to this role in order to aid judgement and decision making skills. Informed decisions need to be based following fact finding including interrogation of systems such as CIS and system1 in addition to discussing cases with healthcare professionals. It is essential that these skills are adopted particularly when dealing with ambiguous or complex cases.

We are looking for individuals with excellent communication skills, both written and verbal, who are able to adjust communication style and content to the audience when dealing with queries from service users and internal/external bodies.

Applicants should also have experience working to deadlines, multi-tasking and be confident using the internet and conducting searches. You should also be competent in the use of Microsoft Office and an awareness of data protection and confidentiality is essential.

Due to the nature of the role; we require candidates to be able to organise and plan to manage their time effectively, have the ability to allocate work and monitor progress independently. You should also have the ability to work without direct supervision, prioritising work, and acting on own initiative where appropriate; pre-empting problems and working to solve them in an appropriate manner. Candidates should also be able to introduce and adapt to change, to enhance efficiency of the work environment.

As SPUR/Gateway is a 24/7 service, we require a candidate that is flexible with working hours, as you may be required to work out of hours.

If you are successful in being offered a position which involves working with patients, you will be required to undertake a Disclosure and Barring Service (DBS) check. The level of check required is dependent upon the position that you have applied for. The Trust will administer the DBS check on your behalf and will recover the cost from your salary. There will be no charge if you are enrolled in the DBS Update Service.
