Post Covid Services – Care Navigator. 

An exciting opportunity has arisen for a Band 4 Care navigator to join Nottingham CityCare’s Post Covid service.  Applications will be welcomed from individuals wishing to work either full or part time.

The post Covid service is a new and exciting development within Nottingham City Care. The service provides essential care and support to those who have on going and long term effects as a direct impact of suffering with COVID-19.

The Service aims to be a proactive, forward thinking and dynamic service which provides evidence based assessment and follow up for individuals requiring on going long covid support and management in the community. The service provides chronic long term management across both health and social care sectors, including the directing and facilitating of additional referrals required whilst providing support to clinicians and professionals in navigating the required services. The team are based at Aspect house but work across a variety of settings in Nottingham city.

We are looking for an enthusiastic, motivated and conscientious team member who is willing to learn and work within a new and growing community Service.

You will be an integral part of the delivery of Multi-Disciplinary Team (MDT) meetings and MDT working. The role includes the identification of patients that may require a health or social care intervention or in complex cases an MDT discussion. As part of the Integrated Care system (ICS), City Care is seeking to recruit Care Navigators who will play a crucial role in establishing integrated health and social care across the City. The role will also involve administrative and data management support to a Multi-Disciplinary Team within the Care Delivery Groups and support on-going improvement in joint working practices.

You will need to demonstrate excellent interpersonal and communication skills.  You must be able to prioritise tasks and manage time effectively, also be able to work in an integrated manner and flexibly within the team and be confident in completing individual tasks, you must also be able to evidence a variety of IT skills.  You will need to hold a valid driving license and have access to a vehicle for this role.

The hours of work will between the hours of 8am and 5pm.
