We are in the incredibly privileged position of caring for people within or close to their own homes. The job of delivering holistic care is incredibly diverse allowing you to constantly develop your clinical skills.

You'll work within one of our Integrated Neighbourhood Teams who offer seamless support and care to our patients. The teams are made up of a wide range of healthcare professionals to deliver nursing, therapy and social care provision. The focus at all times on the individual, either supporting the patient to stay at home, avoid admission or supporting our partners to facilitate hospital discharge. Our service runs 7am to 10pm every day of the year.

There is a statutory requirement for staff in this role to provide nursing or personal care within Care Quality Commission (CQC) regulated care homes.

Therefore, in line with new government regulations which come into force on 11 November 2021, anyone employed in this role will need to evidence they have had two doses of a Medicines and Healthcare products Regulatory Agency (MHRA) approved COVID-19 vaccine, or that they come within a specified exemption. Evidence of vaccination status should be provided via the NHS app, the NHS COVID pass via the NHS website, or an NHS letter as part of pre employment checks

The level of check required is dependent upon the position that you have applied for. The Trust will administer the DBS check on your behalf and will recover the cost from your salary. There will be no charge if you are enrolled in the DBS Update Service.
