Early Interventions in Psychosis and Transitions Service – Early Interventions ARMS Transitions Senior Care Coordinators

37.5 hours/week - 09.00am to 5.00pm

 

An exciting opportunity has arisen for 2 full time ARMS Transitions Senior Care Coordinators. We provide a service to people of Northeast Lincolnshire between the ages of 14 and 35 who may have developed a First Episode of Psychosis (FEP) or be at risk of developing psychosis, At Risk Mental State (ARMS).

We also provide a transition service to young people who are transitioning from CAMHS to Adult mental health services. This is a 1-year service to ensure that these young people have a safe, effective, and seamless transition. As a part of the expansion of the EIP service, two new full time (ARMS Senior Care Coordinators) are required to effectively assess, coordinate and monitor all transitions. You will be working very closely with young people’s services including (YMM) and (Compass Go) and other potential referral sources to enable early detection of young people with predisposed vulnerabilities, who may be at risk of graduating into Adult Mental Health Services. This is a prevention strategy due to an increasing number of complex transition cases such as Out of Area Placements, Frequent Attenders at A&E, Emerging Personality Disorders and Complex Presentations.

You will need to engage clients who are at the beginning of their health journey and due to the stigma of mental health difficulties may be difficult to engage. You will therefore need to have excellent interpersonal communication skills and a good working knowledge of Early Intervention in Psychosis philosophy of care. The Early Intervention in Psychosis and Transitions Service uses a normalising approach to reduce stigma and distress and promote acceptance, tolerance and recovery. As part of this you will be proactive in community outreach with youth forums and activities for our service users. The use of low-level engagement strategies with ensure that no young person slips through net.

This is an exciting time to be joining the team within NAViGO as we are in the process of developing the service, in conjunction with the Local CCG we plan to increase the age range to include 35–65-year-olds in line with the Access and Waiting Time Standards and also increase our capacity to provide NICE compliant care packages. The new posts are to support young people to have access to preventative strategies working close with YMM (CAMHS), Compass and other children services. The age group targeted will be between 14 – 25 and is a part of a new and exciting development working another local provider in NL and NEL.

If you are passionate about working with people at the beginning of their journey in order to get people back on track and living their lives in a meaningful way then this position is for you. We are seeking enthusiastic, highly motivated and passionate professionals to join our dedicated and effective team.

You will need a professional registration as a RNMH, Social Worker or Occupational Therapist to perform this role.

You will be expected to:

  • Act as a Care Coordinator under the Care Programme Approach and manage your caseload effectively
  • Attend regular Training, Managerial and Clinical Supervision
  • Develop skills in Family Interventions and Cognitive Behavioural Therapy
  • Attend Psycho-Social Interventions Training
