The Role:

We have long term placement opportunities available for Band 5 to Band 7 Physio/OTavailable at Northern Care Alliance NHS Group - Radcliffe Primary Care Trust, specifically providing the following:

The Northern Care Alliance is an NHS Group formed by bringing together two NHS Trusts, Salford Royal NHS Foundation Trust and The Pennine Acute Hospitals NHS Trust.

The NCAGroup provides a range of healthcare services including five hospitals and associated community services - Salford Royal, The Royal Oldham Hospital, Fairfield General Hospital in Bury andRochdale Infirmary.

This is a post in a busy Community Physiotherapy/ Occupational Therapy Team. Ideally, we are keen to recruit both an OT and Physio to support the demands on both services for Palliative Care assessment/ intervention.

Palliative Care: Community physiotherapist / Occupational therapist.

  • Work as part of the community physiotherapy / Occupational Therapy team to manage the Therapy needs of patients referred for Palliative Care.
  • To adopt a Multidisciplinary approach to practice, working closely with Nursing and AHP colleagues in the Specialist Palliative care Team.
  • Maintain accurate and contemporaneous notes on electronic and paper systems as required
  • Maintain confidentiality in line with Trust policy
  • You will need to be a car driver with your own transport

Skills:

To apply for this post, you will:

  • Diploma/Degree in OT/Physiotherapy
  • HCPC Registration
  • Appropriate relevant post grad experience working in a community setting
  • Evidence of working in a community team
  • Evidence of working with clients with rehabilitation needs
  • Knowledge of current best practice in rehabilitation
  • Good level of skill in handling patients with rehabilitation needs
  • Ability to undertake appropriate therapy assessments relevant to the clinical specialty
  • Current UK Valid Driving License
  • Appointments to regulated and controlled activities require an enhanced DBS disclosure and registration with the Independent Safeguarding Authority (ISA), Vetting and Barring Scheme

Responsibilities:

  • To carry out appropriate assessment and treatment of allocated patients.
  • To gain consent, undertake assessment, make clinical judgement, and formulate and implement treatment plans with support from Senior Physiotherapists/OT, when required
  • To formulate, implement and evaluate treatment plans in response to assessment and need
  • To work in close liaison with other disciplines involved in the care and treatment of patients allocated to you
  • To provide clinical support to Community Physio/OT Services across specialties. Working flexibly to meet service requirements. This may include Enhanced Intermediate Care / Palliative Care / Rheumatology under the guidance of senior clinicians
  • To be aware of resources / services available to patients within and outside the Physio/Occupational Therapy Service and how to gain access to these; referring on as required
  • To attend appropriate clinical meetings
  • To participate in supervision and training
  • To provide in-service training
  • To maintain strict confidentiality regarding any information concerning patients.
  • To be aware of and adopt correct procedures and departmental policies when treating patients, dealing with visitors, students, and other staff
  • To establish and maintain professional relationships with patients, their relations, and friends and to support and promote patients’ rights and identity
