The post is for 12 months fixed term contract; full time post 37.5 hours per week Monday to Friday.

We are looking to recruit a cancer support worker to join our team, to support patients in the stratified follow up pathway.

Cancer patients are stratified to the appropriate follow up pathway based on clinical and individual needs. Assessment and care planning discussions are held at diagnosis, end of treatment and other key points in the pathway and all patients have a written care plan.

Key responsibility for the cancer support worker would be coordinating and supporting patients in their stratified pathway and ensure that agreed personalised care is followed up appropriately.

The cancer support worker would also ensure accurate recording of data for reporting purposes; and adhere to the service operational policy; as well as provide information/signposting and/or onward referral to the relevant staff or service.

The post is accountable to the team lead for Macmillan Advice an Guidance Service (MAGS) and is required to develop positive relationships with relevant departments and individuals including but not exclusive of MDT coordinators, cancer clinical nurse specialist, radiology departments, outpatient departments and admin staff, members of cancer MDT.
