We are looking for enthusiastic and innovative individuals who possess excellent communication, customer care and IT skill, to act as a clinical screener for referrals into our rehab units.

The candidate will have to be able to prioritise their own workload without supervision, have an excellent telephone manner and be able to work well on their own initiative as well as part of a team.

You will work closely with the discharge HUB leads in the local acute trusts, in improving patient experience, clinical safety and patient flow.

You will be required to screen, clinically reason and accept assessments from a range of referral sources, including inpatient and community services.

We need the applicants to be flexible, approachable and have a sound clinical evidence based background and knowledge. They will need to be friendly but assertive, and be confident in their own decision making, being able to challenge when necessary.

The primary objectives are to support acute services and respond appropriately to ensure a timely turnaround from referral to admission.

The Service will operate between the hours of 8am – 6pm, 7 days per week, and 365 days per year. The successful applicant will be expected to work flexibly throughout this period.

The post holder’s primary base will be Athlone Rehab unit in Westbourne Park, but you will on occasion be expected to undertake face to face clinical screens at other sites, such as St Mary’s, Chelsea and Westminster and University college Hospitals.
