Bolton NHS Foundation Trust was formed in 2008 and, in 2011, we became an integrated provider with community health services joining the trust. Covering a catchment area that also incorporates parts of Bury, Wigan and Salford, we deliver hospital and community based services from the Royal Bolton Hospital in Farnworth and from 20 health centres across Bolton, to a catchment population of 320,000 people. If so, the Health Improvement Practitioner Service (HIPS) are looking for a new Service Manager and we would like to hear from YOU!

 

The HIPS have been an established part of Bolton’s primary care workforce for over a decade. During this time, we’ve helped tens of thousands of patients to prevent illnesses like Type II Diabetes, Cardiovascular Disease and COPD through our 12-month, evidence-based, lifestyle intervention.

 

As our Service Manager you will lead a team of 39 HIPs and 2 Social Prescribing Link Workers across all 49 GP surgeries across Bolton.

 

You will work closely with partners at Bolton Clinical Commissioning Group, Primary Care, Bolton FT, Bolton CVS, Bolton Council and Public Health as well as other stakeholders – so you’ll need to be an excellent communicator at all levels.

 

In addition to helping to improve the service for our patients, partners and team members, you will be responsible for delivering on a range of challenging key performance indicators, in particular supporting primary care in the delivery of the NHS Health Check and other important elements of the Bolton Quality Contract.

 

It is a demanding but rewarding role in what can be a rapidly evolving professional landscape. You’ll be well supported by those around you, but the ability to cope with and lead others through complex change is crucial.
