The Community Health Worker programme will:

  • Deliver targeted health promotion information to families, whilst providing health literacy support by helping residents understand medical advice and recommendations. The roles will also be used as digital champions, highlighting those who are digitally excluded and linking them in with available council support.
  • Help community members use the health care system and access appropriate local services, assisting with appointments and supporting follow up visits to health professionals, whilst identifying and referring patients to appropriate services based on their needs.
  • Support community members through supportive relationships, informal counselling and motivational talks. This is delivered via health coaching, which involves collaborative goal setting, problem solving and action planning. Through the nature of the work, the CHW programme might also be a means of tackling social isolation and loneliness.
  • Deliver needs assessments and providing important information to the community on contextual safeguarding, local picture, asset mapping and needs assessments. It also involves engaging with the community to ensure local health and social services are suitable in both design and delivery.

The Trust has taken the decision to recharge successful candidates for the cost of their DBS disclosure. This post requires a(n) Enhanced / Standard DBS disclosure. The current price of this is £48.43/£30.43. The Trust will pay the initial costs, which will be recovered, in installments, from your first three salary payments.
