Band 5 (Agenda for Change Pay Scale) £24,097 - £30,605

Fixed Term Contract: 2 years

An exciting opportunity is now available in Buckinghamshire, Oxfordshire, and Berkshire West (BOB) Clinical Commissioning Groups for a highly motivated individual to undertake the role of Support Officer for the National Diabetes Prevention Programme.  This role will sit within the Buckinghamshire Long Term Conditions, Prevention and Personalised Care portfolio which is a small and friendly team passionate about the prevention of long-term conditions. 

You will be working with primary care colleagues across Buckinghamshire, Oxfordshire, and Berkshire West so we require an excellent communicator both written and orally. We would require you to be proactive and creative with ways to share information with our stakeholders and colleagues. We are looking for a highly organised individual with excellent administrative and interpersonal skills.  You will be a self-starter who can work independently. 

The role will report into the BOB Project Managers and line-managed by the Bucks Transformation Manager.  Work will include.

  • Liaising with GP practices and providing practical support, advice, and guidance in the delivery of the Diabetes Prevention Programme across BOB
  • Reporting outcomes and evaluation to the Diabetes Programme Project Managers
  • Creating and delivering presentations and reports as required
  • Tracking progress against nationally set targets

This post is full time but flexibility on working pattern can be discussed.  The base is at Amersham or Aylesbury in Buckinghamshire, although in the current climate it will be predominantly home-based initially. Some travel to GP practices will be necessary across Buckinghamshire, Oxfordshire, and Berkshire West.  Interviews will be held on 01.11.21 2021.
