We are seeking to appoint Registered General Nurses as part of our Integrated Rapid Response Service.

This provides an exciting opportunity for those wishing to expand on their assessment and clinical skills within a community environment. The primary goal of our service is to prevent hospital admission and early discharge by supporting clients to return to their home environment. After a comprehensive assessment this may include supporting the client at home through enablement, liaising with social worker colleagues and therapists.. You will also work as part of the clinical hub alongside other members of the Community MDT to ensure that the patient accesses the correct community pathway.

Interested candidates must have excellent assessment and skills and have a willingness to work towards Apaccs, and minor illness qualifications.

You must have excellent communication skills and be able to provide a high standard of nursing care. In addition, you will be able to demonstrate effective interpersonal skills and good knowledge of Inter- Professional working.

Community services is currently undergoing a period of Transformation, the post will require successful candidates to embrace new ideas and concepts, apply an evidence base to your practice and facilitate change if appropriate.

We are seeking pro-active practitioners who will embrace case management and the challenges posed by the current political climate.

A driving licence and access to a vehicle is essential for this post due to the requirement for regular travel.

The cost of undertaking a DBS disclosure will be met by the individual. The Trust will meet the initial costs of the disclosure, and you can choose to have this deducted from either your first month’s salary or in instalments over your first 3 months’ salary. The current price of a standard check is £23 and an enhanced check is £40.
